Important information

• You have 3 months from the date of purchase (indicated in the order confirmation) to undergo all tests in the Package.
• When buying a Package for a child, please enter your details as the ordering party and the child's details in the field: beneficiary.
• To use the services in the Package, you must have a referral from a doctor (paediatrician or other specialist). In the absence of such a referral, prior consultation with a paediatrician at Medicover Hospital will be necessary - such a visit is additionally payable in accordance with the current price list.
• Within 2 business days of purchasing the service on Medistore, your Personal Patient Care Assistant will contact you to suggest a convenient date for the service or to confirm the order.
• Each additional medical procedure ordered by a doctor that is not included in the Package is additionally payable.
• In the event of failure to perform any of the procedures included in the package for reasons attributable to the Patient, the price of the service will not change.
• The package is not subject to discounts for Patients with Medicover subscriptions.
• All the tests, services and medical consultations included in the Package can only be performed at Medicover Hospital in Warsaw. Medicover Medical Centres do not provide the services included in the Package.
