What is gynecological ultrasound? 

Gynecological ultrasound lasts about 15-20 minutes. 

This diagnostic test uses the phenomenon of ultrasound waves reflecting off organ structures – it is a non-invasive, safe, and painless technique. During the ultrasound, the patient may feel slight discomfort. The examination assesses the reproductive organs, including the endometrium, the body of the uterus, the cervix, the ovaries, and the Douglas pouch.

Transvaginal ultrasound is performed using a transvaginal probe protected by a disposable latex cover covered with a special gel. This gel enables high-quality images and a precise assessment of the organs. 

The examination can also be performed through the abdominal wall using an appropriate probe. Gynecological ultrasound through the abdominal wall is primarily used for patients who have not yet started sexual intercourse or have a small opening of the hymen. Alternatively, in some patients, the examination can be performed through the rectum. 

Gynecological ultrasound – types of examinations 

• Transvaginal ultrasound (vaginal) — allows for a detailed evaluation of the reproductive organs by inserting the probe into the vagina. 
• Abdominal ultrasound — used for patients who have not started sexual intercourse or when transvaginal ultrasound is contraindicated. 

Indications for performing ultrasound of the uterus and ovaries 

• bleeding from the genital tract; 
• menstrual irregularities; 
• abdominal and lower abdominal pain; 
• changes in the ovaries, uterus, and endometrium (e.g., polyps, endometrial hyperplasia), ovarian cysts, polycystic ovary syndrome, fibroids; 
• abnormalities in the structure of reproductive organs; 
• infertility; 
• problems with maintaining pregnancy; 
• location of internal contraceptive devices. 

Preparation for the examination 

The requirements vary depending on the method of examination. Below are detailed instructions for both types of examinations. 

Transvaginal ultrasound (vaginal) 

Before the examination, it is recommended to empty the urinary bladder. Also, ensure the hygiene of the intimate areas. 

Gynecological ultrasound through the abdominal wall 

For abdominal ultrasound, the bladder should be full, which helps to visualize the organs better. According to the guidelines of the Polish Society of Gynecologists and Obstetricians, it is recommended to drink 1–1.5 liters of still water about one hour before the examination. 

Recommended timing for gynecological ultrasound

• Women of reproductive age: It is recommended to perform the examination between the 5th and 10th day of the menstrual cycle. 
• Women in the postmenopausal age using hormone replacement therapy: 6-10 days after taking the last pill. 
• Women in the postmenopausal age using HRT in sequential therapy: The examination is recommended up to the 10th day of the cycle.
• Women in the postmenopausal age using HRT in continuous therapy: There are no limitations regarding the timing of the examination.

The gynecologist may adjust the timing of the examination to the individual situation of the patient. In case of improper preparation, the examination may not be performed, or the results may not be reliable.
