Anti-TPO antibodies are antibodies directed against thyroid peroxidase (TPO), a protein that is a component of thyroid tissue and plays a role in the production of thyroid hormones. In healthy individuals, anti-TPO antibodies are not present in the blood. In affected individuals, the immune system begins to recognize thyroid cells as foreign, leading the body to produce anti-TPO antibodies that attack them. Measurement of anti-TPO levels is used in the diagnosis of thyroid and autoimmune diseases. Since an improperly functioning thyroid may not cause specific clinical symptoms, a series of tests is often necessary to establish an accurate diagnosis.

When should the test be performed?

Anti-TPO antibodies may be present in the blood of individuals with autoimmune diseases. Assessment of thyroid function usually begins with testing TSH, FT4, and FT3 levels. If these results are abnormal, anti-TPO testing is recommended.

This test is particularly indicated for individuals who experience symptoms that may suggest thyroid dysfunction, including:

• chronic fatigue,
• unexplained weight gain,
• mood changes,
• impaired thermoregulation,
• dry skin and hair,
• excessive hair loss,
• increased sensitivity to cold or heat.

Anti-TPO testing is also performed in pregnant women to assess the risk of postpartum thyroiditis.

What does the test diagnose?

Anti-TPO testing is performed when autoimmune conditions are suspected, such as Hashimoto’s thyroiditis, rheumatoid arthritis, Graves’ disease, systemic lupus erythematosus, or pernicious anemia.

Autoimmune diseases are characterized by the presence of autoantibodies against TPO. The presence of anti-TPO antibodies is a hallmark of Hashimoto’s disease. However, a positive result does not definitively confirm autoimmune thyroiditis, and a negative result does not exclude it.

Anti-TPO – preparation for the test

• The test involves drawing blood from a vein, which is then sent to a laboratory for analysis.
• Blood collection takes several to a dozen seconds.
• No special preparation is required for the test; fasting is not necessary.
• Blood should be collected in the morning hours.

Anti-TPO reference values

There are no strictly defined normal reference values for anti-TPO test results. Each result is assessed individually by qualified medical professionals, taking into account the patient’s sex, age, and overall health condition.

It is important to discuss every anti-TPO test result with a physician.
